Checklist
For the issuance of, in particular, a general power of attorney and precautionary health care power of attorney with advance care directive/living will, the notary will need the following information:
- Full name, date of birth and address of the authorized representative(s).
- For several authorized representatives: Should these persons be authorized to act individually or jointly?
- Should the authorized representatives immediately be able to act on your behalf, or only if you are unable to take care of yourself?
- For advance care directives/living wills: Are you in agreement with an organ donation?
