Checklist

For the issuance of, in particular, a general power of attorney and precautionary health care power of attorney with advance care directive/living will, the notary will need the following information:

  1. Full name, date of birth and address of the authorized representative(s).
  2. For several authorized representatives: Should these persons be authorized to act individually or jointly?
  3. Should the authorized representatives immediately be able to act on your behalf, or only if you are unable to take care of yourself?
  4. For advance care directives/living wills: Are you in agreement with an organ donation?
